Sensorum Health has entered into a strategic partnership with Intermountain Health, a leading health system, to improve health outcomes and primary care access to older adults living independently with multiple chronic conditions. The partnership is focused on helping older adults with chronic conditions stay out of the hospital and healthy at home, but for patients with chronic disease, this becomes challenging. Sensorum provides advanced remote monitoring technology and integrated clinical staff to identify and triage the earliest signs of health decline.
