The 2024 Community Health Needs Assessment is designed to help healthcare professionals better understand the needs and issues of people in the area to help address them. Those living in Douglas, Sarpy or Cass Counties in Nebraska and Pottawattamie County, Iowa, are invited to participate online or by phone both in English and Spanish. The online survey should take about 25 minutes to complete.
